Frequently Asked Questions — Medicine

What is the medicine training pathway in the UK?
Internal Medicine Training (IMT) runs from IMT1–IMT3 (replacing the old CMT). After IMT, trainees apply for higher specialty training (ST4+) in a subspecialty such as cardiology, gastroenterology, respiratory, etc. The full pathway leads to a CCT and consultant appointment.
What exams do medical trainees need to pass?
The MRCP(UK) (Membership of the Royal College of Physicians) is essential. It has three parts: MRCP Part 1 (MCQ), MRCP Part 2 (written), and PACES (clinical examination OSCE). Part 1 is often attempted during Foundation training. All parts must be passed before applying for ST4.
How competitive is medical training in the UK?
IMT has become increasingly competitive. The 2025 CT1 competition ratio was 5.27:1. Higher specialty training varies widely — cardiology and gastroenterology are very competitive, while some subspecialties are less so. Full MRCP, relevant audit, and teaching experience strengthen applications.
What courses help medical trainees?
Essential courses include MRCP revision courses (Part 1, Part 2, and PACES), clinical examination skills workshops, ALS certification, acute medicine courses, and subspecialty-specific revision programmes. Question banks are particularly valuable for MRCP Part 1 and Part 2.
